DRIVING HISTORY

Do you hold a valid Ontario Class G or standard Quebec provincial driver’s license? *

Are you able to obtain a Provincial Ministry of Transport Driver Record Search - No older than 30 days (from date of submission)? Maximum demerit points allowed is 5 *

Are you able to obtain a Police Records Check - For Service with The Vulnerable Sector (Form 306) - No older than 90 days from date of submission? *

Are you able to obtain a Claims experience letter from previous and present car insurer? *

Do you have a Social Insurance Card (plus Work Permit if SIN card starts with 9 0 0)? *
